The school counselor is the coordinator of 504 plans.
A 504 plan is a legal document falling under the provisions of the Rehabilitation Act of 1973. It is designed to plan a program of accommodations to assist students with special needs who are in a regular education setting. A 504 plan is not an Individualized Education Program (IEP) as is required for special education students.
The school counselor is also the I&RS coordinator.
I&RS is a state mandated system of clearly defined steps that are taken to assist a struggling student in improving educational outcomes. These steps are taken by a team, which meets and monitors the success of students identified through the referral process, as needing academic and/or behavior interventions in order to be successful. These interventions are monitored over a period of time and may be modified, enhanced or discontinued as indicated by the needs of the student involved.

- Q. What is the process for placing a student on a 504 plan?
A. There are four steps:- Student is referred by teacher, support staff, parent/legal guardian, physician, or therapist.
- A 504 plan meeting is held.
- A plan for the student is developed.
- A review date is set.
- Please note- a diagnosis from a medical doctor is required to have a 504 plan put into place.
